100 Year Lifestyle assessment

Fill out the assessment and bring to the 100 year lifestyle lecture. Your guest can
fill one out too. We will go over what the score means that night. If you can not
make it to the weigh in, email us your score and we will send you the results after
the 100 Year lifestyle lecture.

Fill out the food journal for each day. We would like you to do 7 days. Don't worry about
counting calories or fat/carbs/sugars, just write down the food like:
Grilled Chicken breast on sourdough with fruit and vegetables.

Goals:
All of yours goals must contain;
(1) time-frame, (2) action step and (3) be measurable.
Short term goals should be for each of the 5 weigh ins,
Long term – by April 10th and
Lifetime – what you want to maintain years down the road.
